Student Emergency Medical (High School Students)   Student Emergency Medical (All Other Students)   Student Appropriate Use   Photo Release Notification Please note: families must fill in each year, even if you completed one in previous years.

For Athletes:

Athletic Emergency Medical Form Required for athletic participation each school year.   Athletic Code of Conduct Required for athletic participation each school year.   OHSAA - Physical Evaluation Required for athletic participation.  Valid for one year from date of exam.   Athletic Injury Waiver Required for athletic participation each school year.   Alternate Transportation Slip Required for alternate/emergency transportation after an athletic event.

 

For Staff:
